SUMMARY

We are seeking a part-time Care Services Coordinator to join our team to provide care services, education and advocacy to families living with ALS. 

ESSENTIAL JOB RESPONSIBILITIES:

Core duties and responsibilities include the following. Other duties may be assigned.

Supports clients and families impacted by ALS

  • Respond to clients and family inquiries for information on ALS and/or community resources.
  • Provide home visits with ALS/PLS clients.
  • Assist clients in registering with the ALS Association as well as the ALS National Registry.
  • Educate clients and families about the mission of the ALS Association and the latest information about research, advocacy and care services.
  • Facilitate access to chapter services, resources such as Durable Medical Equipment, Assistive Technology, Educational Material and other community resources.
  • Consistently maintains communication with ALS clients and their families.
  • Maintain accurate records, files and databases, providing reports as required or requested.
  • Understand that only certified medical professionals may provide advice and opinions related to medication, treatment and treatment plans to persons with ALS.

Coordinate Multiple Care Service Programs

  • Provide support to clients and clinic team as the community liaison.
  • Responds to and coordinates requests for ALS Loan Closet equipment.
  • Facilitates monthly support group meetings and schedules speakers throughout the year to provide education to attendees.

Support Professional Outreach Program

  • Initiate and cultivate relationships with ALS-related healthcare service providers.
  • Provide professional in-service educational programs to healthcare professionals, e.g; Home Health agencies, Durable Medical Equipment (DME) providers, Skilled Nursing Facilities (SNFs) and Hospice Organizations.
  • Serve persons with ALS and their families by providing clinic support, resources and referrals and/or facilitating education and support group meetings across the Nationally Managed Chapter and Service Area network. Other responsibilities may be assigned as necessary to support our mission in the communities we serve.
  • Support achievement of, at a minimum, the Expanded level of the Pathways to Excellence document that serves as a road map in Care Services program growth.
  • Convey a professional and positive image that reflects favorably on The ALS Association.

QUALIFICATIONS:

  • Minimum BA/BS degree in social work or another health/human service-related field with a minimum of three years professional experience required.
  • Ability to establish and maintain professional relationships with clients, volunteers, human service professionals, the public and co-workers.
  • Strong organizational skills and communication skills, including computer skills.
  • Ability to function independently within set guidelines.
  • Works with other chapter staff as a team member to provide effective services for people impacted by ALS.
  • A strong work ethic, essential for this demanding environment that continually strives for excellence.
  • Ability to drive locally, travel within assigned territory and work occasional evenings and/or weekends for Association business and events. Ability to travel nationally on Association business as required.
  • Computer skills required: Accounting Software; Development Software; Human Resource Systems; Inventory Software; Microsoft Office; Payroll Systems; Project Management Software. Familiarity with ADP Workforce Now, Concur and Salesforce a plus.
