ID: 76219321 Shift: 8H Days 8 hr days 7-4:30 Description: Unit Notes:50 mile radius rule for travelersWill accept LOCAL @ $70FLU VACCINE IS REQUIRED at UMMC - only medical exemption is allowed and requires documented evidence from a health care provider of a past severe reaction to the vaccineFINGERPRINT REQUIREMENT - Remote process must allow 10 days, In Person can be done at Jackson or Grenada campus the week prior to startTHIS HIRING MANAGER IS LOOKING FOR HIGHLY EXPERIENCED CANDIDATES WITH ACUTE INPATIENT CASE MANAGEMENT AND DISCHARGE PLANNING EXPERIENCE Training on unit will be minimal - candidates will need to be able to pick up quickly and work efficiently. REQUIRED: - Minimum 3 years of Acute Inpatient Case Management experience that includes at least 1 year in a LARGE Hospital (500 beds, LEVEL 1/ACADEMIC Medical Center) (this does NOT include telephonic, utilization management, or therapy experience), - Strong computer skills, Epic EMR experience, - Nursing degree Prefer RN, BSN, *Would consider ADN .BLS certification through AHA, CCM preferred but not required. - Acute discharge planning experience, Recent (within the last two years) acute inpatient case management experience. Shifts & Scheduling ?M-F 8-430p Weekend RequirementsNone Holiday Requirements Rotated as needed Case load/ patients per day/consults per day 15-20 Will cover all Inpatient units throughout the hospital RN's work with Social Work team which consists of 15 full-time and 17 part-time Social workers.UMMC has contracts with 5 local facilities with 10 beds reserved at each facility to assist with the complex placements, however specific criteria must still be met. Criteria has a limitation on financial assistance for only those uninsured or underinsured. UMMC is the only level 1 trauma hospital in the state so will receive a variety of complex patients and those with no other hospital options.Scrub Color/Dress CodeAny color scrubs or business professional attire RT for DC Planning?Will consult with RT as need Pharmacy for DC Planning?Will consult with Pharmacy as needed Department Specific Orientation2 weeks (will include general facility orientation)Will work alongside CM team so will never be working alone.
