Job Description:
AccentCare is seeking a dedicated and compassionate Patient Care
Manager / Registered Nurse to join our home health care team. In
this role, you will oversee the daily clinical operations and
coordination of patient care in the home setting, ensuring high
standards of care are met consistently. You will be instrumental in
developing care plans, managing a team of nursing professionals,
and maintaining compliance with federal and state regulations. This
position offers the opportunity to work closely with patients and
their families, providing not just medical care but also support
and education to improve their health and quality of life.
Supervise and organize a team of home health nurses and other
healthcare professionals. Develop and implement comprehensive
patient care plans in collaboration with team members. Ensure all
patient care provided adheres to state, federal, and organizational
standards. Provide education and support to patients and their
families regarding health conditions and home care techniques.
Actively participate in the hiring, training, and evaluation of
staff members. Maintain accurate patient records and submit timely
reports on patient status and care. Coordinate with physicians and
other healthcare providers to ensure the continuity of patient
care. Strong leadership and team management skills Excellent
understanding of healthcare regulations and policies Effective
communication and interpersonal skills Proficiency in clinical
assessment and care planning Ability to handle stressful situations
and make informed decisions Skilled in the use of medical software
and record-keeping Medical Insurance Dental Insurance Vision
Insurance 401k Current Registered Nurse (RN) license in the state
of practice CPR certification Valid driver’s license and reliable
transportation Preferred: Certification in Healthcare Management or
a related field
