AccentCare is inviting a dedicated Physical Therapist to join our home health team, where we strive to improve the lives of our clients through personalized and compassionate care. As a Physical Therapist in our network, you will implement high-quality physical therapy services for patients in their homes. Your expertise in assessing patient needs, developing, and managing therapy plans will directly contribute to enhancing their physical health and improving their daily lives. This role requires a deep commitment to patient-centered care and the ability to operate independently in a dynamic home health environment. Assess patient condition by conducting physical exams and obtaining medical/functional history. Develop and implement therapy plans tailored to individual patient needs in collaboration with other healthcare professionals. Document patient care services by charting in patient and department records in accordance with regulatory requirements. Advise patients and their families on techniques for in-home health care and disease prevention. Evaluate the effects of therapy treatment and communicate patients' progress to relevant stakeholders. Participate in ongoing education to maintain licensure and to stay updated with current physical therapy practices. Strong expertise in physical therapy modalities and assessments Excellent patient management skills and empathetic communication Ability to design practical, creative treatment programs in a home health setting Proficient in using healthcare documentation software Medical Insurance Dental Insurance Vision Insurance 401k Valid state Physical Therapy license CPR certified Driver's license and reliable transportation
