Registered Nurse (RN) | Case Manager Location: Yazoo City, MS Agency: RadGov, Inc Pay: $2,400 to $2,500 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position TravelNurseSource is working with RadGov, Inc to find a qualified Case Manager RN in Yazoo City, Mississippi, 39194! Job Location: Yazoo City, MS Job Duration: 13weeks GTDW: 40hrs Shift: Day 5x8-Hour (08:00AM - 05:00PM) LOCAL PAY: $50-$55/HR TRAVELER WEEKLY PAY: Weekly Tax Free Stipend to be paid: $1,162.00 Weekly Straight Pay (Taxable): $1,338.00 Weekly Gross: $2,500.00 Description: Primary Responsibilities: Perform comprehensive assessment of high risk members. Collaborate with primary care providers to ensure the implementation of an individualized, comprehensive care plan with specific interventions designed to engage the member. Address, identify, and continuously reassess cost-efficient, appropriate levels of care. Put non medical support in place to ensure compliance with treatment plans (such as housing and transportation). Coordinate care transitions as needed. Coordinate with physicians, social workers, discharge planners, and claims professionals in transitioning patient to appropriate level of care. Engage specialty resources as needed to achieve optimal resolution. Keep detailed records of clinical, functional, and fiscal outcomes during the management process. Identify opportunities for health promotion and illness prevention. Prevent adverse patient occurrences when possible and intervene quickly if prevention is not possible, thereby minimizing poor outcomes.
