General Description Under the direction of the Director of Nursing, the Shift Supervisor/ Relief Supervisor is responsible for maintaining the standards of nursing care andpractices within the Facility to meet the physical, psychosocial, and rehabilitation needsof the residents.Essential Duties1. Perform various duties to provide quality nursing care to residents to maintain orattain the highest practical level of functioning and to coordinate total nursing care for theresidents as illustrated by the following:a. Assesses residents who have had an accident or incident that results ininjury or any unusual event. Documents findings as per Resident CareManual Policy.b. Thoroughly investigates allegations of alleged abuse / neglect ormisappropriation of property and notifies appropriate parties as perpolicy.c. Makes daily rounds in areas of nursing care under his/her supervision,visiting each unit to evaluate overall unit performance, and to receivecomments relating to the residents needs and problems, and implementnecessary changes.d. Delegates responsibility to nursing personnel through workassignments, etc. for direct nursing and non-nursing care of patientsduring the shift.e. Assists in assessing level and number of personnel assigned to the unit.Makes recommendations regarding such personnel where indicated forbetter patient care and utilization of such personnel.f. Supervises and evaluates the performance of nursing personnel underhis/her direct supervision.g. Instructs nursing personnel on new policies and procedures andtreatments to allow for the most effective use of nursing time.h. Reports status of nursing personnel and other supervisory matters to theAssistant Director of Nursing Services and/or Director of NursingServices, on a daily basis.i. Makes recommendations for hiring, disciplining, evaluating and thetermination of personnel through written reports and verbalcommunication.j. Completes appropriate evaluation summaries and assists with thosecompleted by Unit Managers and/or Charge Nurses, as needed.k. Assists in the scheduling of personnel to adequately cover all nursingshifts.l. Assists with the preparation of the nursing department budget andadheres to that budget as approved by the Executive Director.m. Assesses, monitors, and treats specific patient care conditions pursuantto physician orders.n. Maintains open line of communication with attending physicianregarding patient care and/or treatment issues.o. Verifies the coordination of nursing care with other departments anddisciplines within the Facility to best meet resident care objectives.p. Conducts periodic rounds to observe Facility appearance and safety,and makes recommendations or assignments for nursing personnel inregard to these concerns and/or report these concerns to the appropriatepersonnel.q. Coordinates the resolution of resident activity, social service, andnutritional problems observed by nursing personnel.2. Establishes and maintains resident care systems per standardized procedure thatare consistent throughout the Facility. These include:a. Unit Management System for group assignments and nursing assistantresponsibilities.b. Restraint Reduction System to verify restraints are at a minimum, and thatrestraints are released at least every two hours, or per Care Plan. Restraintreleases are to occur on the unit at the same time or per Care Plan, and beobserved by a licensed staff member.c. Turning and Positioning System to verify schedules are followed everytwo hours or per Care Plan.d. Resident Ambulation System to verify effectiveness and adherence toresident ambulation program.e. Scheduling System to verify scheduling for nurses and cna's is completedin a fair and consistent manner.f. Nourishment/Supplements System to verify nourishments/supplements arepassed according to developed schedule.g. Cleanliness System to verify that environment is clean, orderly andorganized. Reviews written cleaning schedules.h. Infection Control System to verify residents are not at risk for infection.i. Pharmacy System to verify correct meds are received timely, meds arediscarded timely and properly and costs are appropriate for resident payersources; and med room and med carts are monitored for policycompliance.j. Lab System to verify labs are drawn timely, and in accordance withdoctors orders.Standard Requirements1. Supports the Mission, Values and Vision of the Facility.2. Is knowledgeable of resident rights and supports an atmosphere which allows forthe privacy, dignity and well-being of all residents in a safe, secure environment.3. Supports, cooperates with, and implements specific procedures and programs for:OSHA and safety, including standard precautions and safe work practices,established fire/safety/disaster plans, risk management, and security, report and/orcorrect unsafe working conditions, equipment repair and maintenance needs.4. Adheres to:a. Confidentiality of all data, including resident, employee and operationsdata.b. HIPAA regulationsc. Quality Assurance requirements and compliance with regulatoryrequirements.d. Compliance with current law and policy to provide a work environmentfree from sexual harassment and illegal and discriminatory behavior.e. Code of Conduct and Compliance Programs when performing jobfunctions5. Supports and participates in common teamwork:a. Cooperates and works together with all co-workers; plans and completesjob duties with minimal supervisory direction, including appropriatejudgment.b. Uses tactful, appropriate communications in sensitive and emotionalsituations.c. Follows up as appropriate with supervisor, co-workers or residentsregarding reported complaints, problems and concerns.d. Promotes positive public relations with residents, family members andguests.e. Completes requirements for in-service training, acceptable attendance,uniform and dress codes including personal hygiene, and other workduties as assigned.f. Participates as assigned in the orientation of new staff and demonstrates apositive, helpful, and enthusiastic attitude.6. Immediately reports incidents of alleged resident abuse or neglect or allegedviolations of residents' rights to Director of Nursing or Executive Director.7. Knowledgeable of age specific developmental factors specific to adult andgeriatric residents (i.e. physical, cognitive and socialization factors) in planningdelivery of care.8. Maintains current practice standards or changes by participating in educationalopportunities provided by the Facility and outside resources.Other DutiesProvides hands on nursing care to residents as needed as a registered professional nurse,consistent with the state's Nurse Practice Act.Perform tasks which are supportive in nature to the essential functions of the job, butwhich may be altered or redesigned depending upon individual circumstances. Performsother work duties as assigned.QualificationsEducation/Training:Maintains a current, valid License as a registered professional nurse in good standing.Must have current CPR certification.A.D.A ((Americans with Disabilities Act)(as amended)) REQUIREMENTS: Must beable to bend, stoop, push, pull, and walk and stand for most of the working day. Requiresmanual dexterity sufficient to provide hands-on care to residents as well as to operatestandard office machines such as fax, telephone, etc. Requires normal range of hearingand vision. Must be free from communicable diseases that pose a "direct threat" to thehealth or safety of others and which cannot be eliminated by a reasonableaccommodation. Must be able to lift a minimum of 50 pounds, and transfer and transportresidents. Must be able to carry and transport supplies. Must be able to pass a post-offer,pre-employment physical health examination as a condition of employment. Must becapable of performing the essential functions of the job, with or without reasonableaccommodation(s).Skill(s): Proficient reading, writing, grammar, and mathematics skills; proficientinterpersonal relations and communicative skills; knowledge of drug interactions, sideeffects, and incompatibilities; decision-making skill; proficient assessment skills; safetytechniques; basic computer skills; able to operate various medical and office equipment,auditory and visual skills.
